What information will I need to provide in order to obtain a life insurance policy?

You will need to prove insurability through the application process and may be required to fulfill medical testing based on age and insurance amount requirements. Your current and historical medical status as well as minimal information on family history may also be required. All information is kept highly confidential.

BF&M is also compliant with the “know your customer” process so we are also required to obtain proof of residency documents including a current utility bill and verification of identity.
